AFTER SCHOOL FORMS

As a participant, I agree to follow this Code of Conduct and help create a respectful, safe, focused, and supportive learning environment. I commit to:

• Attend class consistently, arrive on time, and remain engaged for the full session each day.
• Students’ arrival time is 2:30pm to 5:30pm. No lingering around the school grounds is permitted at the end of class.
• Students are not permitted to leave school grounds to purchase food or snacks before class begins. M-DCPS will be providing a snack each day. If a student violates this policy, the School Principal has the right to dismiss the student from the program, and the student can face other actions per school policies.
• Complete assigned production tasks during and when required in class.
• Participate in scheduled filming days.
• Attend the film premiere screening, with the date to be announced.
• Treat myself, fellow students, staff, volunteers, guests, equipment, and facilities with respect.
• Always use appropriate language and behavior during class and production activities.
• Follow directions from the staff and ask for help when I need clarification or support.
• Be open to new experiences, creative challenges, collaboration, and constructive feedback.
• Complete all required mid-program and post-program surveys by the deadlines requested. Failure to do so will impact your Community Service Hours.

PARENT / GUARDIAN ACKNOWLEDGEMENT AND SUPPORT COMMITMENT

By signing below, I acknowledge the student Code of Conduct and agree to support my child’s successful participation in the 2026 -2027 FFI- After- School CINEMA Program.

• Avoiding scheduling activities that conflict with required class sessions, filming days, or program commitments.
• Ensuring my child attends regularly, arrives on time, and remains on site until the end of each session.
• Ensuring my child does not leave school grounds to purchase food or snacks before class begins and understands that M-DCPS will provide a snack each day.
• Understanding that if my child violates the school-grounds policy, the School Principal has the right to dismiss my child from the program and may take additional action in accordance with school policies.
• Encouraging my child to complete production tasks and communicate with staff if support is needed.
• Supporting respectful conduct toward students, staff, volunteers, guests, equipment, and facilities.
• Attend the film premiere screening, with the date to be announced.
• Complete all required mid-program and post-program evaluation surveys by the deadlines requested. Failure to do so will impact your child’s Community Service Hours.

Any such Recordings may reveal your identity through the image itself without any compensation to you, your children, or wards.

Any and all Recordings taken of you, your children or wards shall be the sole property of Florida Film Institute, Inc., and its funders.

With regard to the use of any Recordings taken of you, your children, or wards, you hereby waive any and all present and future claims you may have against Florida Film Institute, Inc their staff, service providers, employees, agents, affiliates and Board members.

FFI and its funders retain exclusive intellectual property rights to all scripts and films produced through the 2026–2027 After-School CINEMA Program. If any program materials are posted on any social media platform, website, or other online forum without prior written permission from FFI, the film may be immediately disqualified from current and future film festival consideration.

Each year, FFI hopes that many of the films created by students will be eligible for consideration by local, state, and national student film festivals. Many film festivals have rules that prohibit submissions if the film has already been shared publicly through television, cable channels, social media, or other websites.

Students may be eager to upload, download, or share projects they have created. However, projects completed during participation in FFI programs may not be shared publicly without written permission from FFI.

REQUEST FOR MEDICATION INFORMATION

To ensure the safety of your child at FFI’s Summer Film Camp CINEMA Program, we are requesting that you complete the following Food Allergy/Severe Food Allergy Information. This form allows you to disclose whether your child has a food allergy or severe food allergy that you believe should be disclosed to FFI in order to enable us to take necessary precautions for your child’s safety. Please list any foods to which your child is allergic or severely allergic, as well as the nature of your child’s allergic reaction to the food.
